6. Survey Sweepstakes Terms & Conditions (Expired Offer)

1. Eligibility

Open to legal residents of the United States and District of Columbia who are 18 years of age or older at the time of entry. Employees, officers, and directors of JOHN HARDY USA INC., its affiliates, subsidiaries, advertising, promotion, and fulfillment agencies, and their immediate family members and persons living in the same household are not eligible. Void where prohibited by law.

2. Sponsor

The sweepstakes is sponsored by JOHN HARDY, INC., 429 Broadway, Floor 3, New York, NY 10012.

3. How to Enter

Complete the JOHN HARDY, INC. survey during the promotion period and provide valid contact information to be entered into the sweepstakes. Limit one entry per person. Entries generated by script, macro, or other automated means are void.

4. Promotion Period

The sweepstakes begins on 9/19/2025 12:00AM ET and ends on 10/31/2025 11:59PM ET (“Promotion Period”). All entries must be received by 10/31/2025 11:59PM ET.

5. Prize

One (1) winner will receive a $200 American Express Gift Card. Approximate Retail Value (ARV): $200. Prize is non-transferable and no substitution will be made except at Sponsor’s sole discretion. Gift card is subject to issuer terms and conditions.

6. Winner Selection and Notification

One (1) potential winner will be selected in a random drawing from all eligible entries received, to be conducted within 14 days after the Promotion Period ends. Odds of winning depend on the number of eligible entries received. The potential winner will be notified via email and phone within 7 days of the drawing. If a potential winner does not respond within 7 days of notification, or if eligibility requirements are not met, an alternate winner may be selected.

7. Publicity Release

Except where prohibited, participation constitutes winner’s consent for Sponsor to use winner’s name, likeness, and entry for promotional purposes in any media without further payment or consideration.

8. General Conditions

Sponsor reserves the right to cancel, suspend, or modify the sweepstakes if fraud, technical failures, or any other factor beyond Sponsor’s reasonable control impairs the integrity of the promotion. Sponsor is not responsible for lost, late, incomplete, or misdirected entries.

9. Release and Limitations of Liability

By participating, entrants agree to release and hold harmless Sponsor, affiliates, subsidiaries, and advertising/promotion agencies from and against any claim or cause of action arising out of participation in the sweepstakes or receipt/use of any prize.

10. Governing Law

The sweepstakes and these Official Rules are governed by the laws of the State of [insert state], without regard to conflict of law principles.
